About us
Survivor Fitness is operated by Mayhem Fitness Pty Ltd (ABN 28 621 760 389) at Shop 1, 126 Soldiers Road, Bowen QLD 4805. We handle personal information in accordance with applicable Australian privacy laws, including the Privacy Act 1988 (Cth) and the Australian Privacy Principles where they apply.
Information we collect
We may collect your contact and emergency-contact details, date of birth, membership and access records, payment information, communications with us, pre-exercise screening responses and relevant health, injury, disability or accessibility information. We may also collect CCTV images and basic website-use information.
How we use and disclose information
We use information to respond to enquiries, manage memberships and payments, provide gym access and requested services, support member safety, communicate with members, investigate incidents and meet our legal obligations.
Information may be shared with authorised staff and service providers where necessary, including GymMaster, EzyPay, technology providers, insurers and professional advisers. It may also be disclosed to police, emergency services, regulators or other parties where authorised or required by law, or where reasonably necessary to respond to an incident or legal claim. We do not sell personal information.
CCTV and access monitoring
CCTV may operate at entrances, on the gym floor, in common areas and around external access points for safety, security, access control and incident investigation. Cameras will not be installed in toilets, showers or private changing areas, and signage will identify monitored areas. CCTV is intended to record video images only, with audio recording disabled.
Footage and access records are restricted to authorised persons and retained only for as long as reasonably necessary or required by law.
Marketing communications
We may send gym news, offers and membership information by email or text where you have consented or where otherwise permitted. You may unsubscribe using the link in an email, replying STOP to a text where available, or contacting us. Essential membership, payment, safety and access messages may still be sent.
Security, access and correction
We take reasonable steps to protect personal information from loss, misuse and unauthorised access or disclosure. You may ask to access or correct personal information we hold about you. We may need to verify your identity before completing a request.

Contact our Privacy Officer at info@survivorfitness.com.au or write to Survivor Fitness, Shop 1, 126 Soldiers Road, Bowen QLD 4805. We will investigate and respond within a reasonable period. If you are not satisfied with our response, you may contact the Office of the Australian Information Commissioner at oaic.gov.au.
